Inclusion criteria

Inclusion criteria: - elective total hip arthroplasty (THA) or total knee arthroplasty (TKA) - general anesthesia with or without peripheral nerve block

Exclusion criteria

Exclusion criteria: - known psychiatric disorders (psychosis, major depression) or other cognitive impairment - language barrier or impaired hearing - no legal competence

Incidence of postoperative delirium or neurocognitive disorders - Testing neurocognitive disorders (MoCA-Test preoperative, day of surgery, 1.-5. and 90. postoperative day)

Secondary

Evaluation of risk factors of a postoperative delirium - postoperative pain by NRS-Scale (2h postoperative, 6h postoperative, 22pm on operation day, 1.-5. postoperative day in the morning and in the evening) - anticholinergic medication - Preoperative evaluation of heart failure (nt-proBNP preoperative) - intraoperative risk factors (blood loss, clotting factors, medication, cerebral oximetry) - biomarkers as an indication of neuroinflammation, blood-brain barrier damage or neuronal damage (e.g. protein S100B, NT-proCNP or systemic inflammation parameters). - biomarkers of kidney injury (proEnkephalin, TIMP-2, IGFBP-7) Evaluation of time of reconvalescence after total hip arthroplasty or total knee arthroplasty - LUCAS-Index preoperative - Barthel-Index (ADL-Score) preoperative, 1.-5. postoperative day - record of organ dysfunction 1.-5. postoperative day
