Inclusion criteria

Inclusion criteria: - at least 3 days of inpatient treatment - discharge to care at home - fluent in the german language

Exclusion criteria

Exclusion criteria: - Patients who have been discharged for further inpatient treatment. - Patients who cannot provide self-assessment due to their health situation
