Inclusion criteria
Inclusion criteria: patient of inpatient geriatric rehabilitation; member of AOK Baden-Württemberg as health insurance provider; at least independent mobility at room level; accessibility for intervention staff (residence < 30 km to the study center);
Exclusion criteria
Exclusion criteria: terminal/fatal disease (life expectancy < 6 months); institutional care after discharge from inpatient rehabilitation (long-term, respite, or short-term care); outpatient/mobile rehabilitation following inpatient rehabilitation; planned inpatient hospital stay in the first 12 weeks after discharge from inpatient rehabilitation; moderate to severe cognitive impairment (Mini Mental Status Examination, MMSE < 21 points); insufficient communication skills;
