Inclusion criteria: Active-motivated expression of interest for preventive behaviour change

Exclusion criteria

Exclusion criteria: 1. Level of care >3 according to the German Social Welfare Code Book XI (SGB-XI) 2. Participant is at the end-of-life

1. General health status via structured interview with WHODAS 2.0 at T 0 and T2 after 12 months

1. Health-related quality of life via structured interview with EQ-5D VAS at T0, T1 after 6 months and T2 after 12 months 2.
