Inclusion criteria

Inclusion criteria: Neurologists in private practice agree to participate in the questionnaire-based collection of study data before and after the study, to transmit teleconsultations via KIM (communication in medicine; secure e-mail and data exchange within telematics) to Ruhr Epileptology for the evaluation of cell phone videos of seizure-like health disorders, and to issue TANs and information material about vCreateNeuro to patients with seizure-like health disorders in vCreate. If KIM is not yet available for neurologists in private practice, a referral slip can be issued as an alternative. Neurologists in private practice agree to discuss the findings of the tele-consult with the respective patient (which will not be documented and evaluated in the study). Physicians of the Epilepsy Reference Center agree to participate in the questionnaire-based collection of study data before and after the study.

Exclusion criteria

Exclusion criteria: Neurologists in private practice or physicians at the Epilepsy Reference Center do not agree to participate in the study or are not willing to transmit teleconsults and inform patients about the use of vCreate Neuro to transfer videos and video-related clinical data.

Design outcomes

Primary

MeasureTime frame
Is the (organizational) establishment of teleconsultations for the reference reporting of videos of seizure-like events between neurologists in private practice and a hospital-based epilepsy reference center successful? The frequency of successfully completed video reports sent to the referring physicians during the study period will be determined in comparison to the number of neurologists in private practice and the number of consilience requests sent by them to the epilepsy reference center.

Secondary

MeasureTime frame
- Retrospective analysis of the current situation before the study from the perspective of neurologists in private practice and the epilepsy reference center regarding o differential diagnostic safety regarding seizure-like health disorders with and without available cell phone videos o Possibilities of using a telemedical reference diagnosis of cell phone videos of seizure-like health disorders o Experiences and assessments of neurologists in private practice and epilepsy reference center regarding intersectoral teleconsultations o Anticipation of the frequency of use of teleconsultations for telemedical reference diagnosis of cell phone videos of seizure-like health disorders o Experiences and assessments of neurologists in private practice and the epilepsy reference center with regard to the digitalization process of the health care system. - Analysis of the same points after the introduction of teleconsultations between neurologists in private practice and Ruhr-Epileptology - How many teleconsults are issued over the study period of 6 months (absolute and in comparison of first to third third; comparison with previously expressed expectation)? In addition, the difference between TANs issued and videos received by the reference center will be determined. - Analysis of the reimbursement options (statutory health insurance, private health insurance) for the aforementioned steps on the part of the neurologists in private practice and on the part of the epilepsy reference center with regard to the reference findings of the transmitted videos. - Health economic consideration or modeling: can an established system of DSGVO-compliant video transfers achieve cost avoidance effects? - Analysis of the legal aspects of the teleconsilium instrument using the vCreate Neuro platform for reporting cell phone videos of seizure-like health disorders from the perspective of patients, neurologists in private practice and the epilepsy reference center. - Analy
