Interventions

Group 1: Adult, not-too-frail, primary care patients receive a cost-free facilitation for creating an advance directive at their primary care physician's practice.

Inclusion criteria

Inclusion criteria: GP patients aged 18 years and older with a frailty score of six or less.

Exclusion criteria

Exclusion criteria: • not able to give consent • Ill with limited life expectancy with a frailty score of seven to nine • Permanent residence in a care facility • insufficient language skills for participation

1) Description of expectations of, and experiences with, the consultation process of Advance Care Planning Germany through a longitudinal qualitative interview study. 2) Description of quantitative results documented in resulting advance directives.
