Inclusion criteria
Inclusion criteria: - Adults: = 18-year-old patients - Self-referral in one of the participating ERs - Feasibility of the SmED-Kontakt+ assessment (sufficient German language skills, possible verbal communication, sufficient cognitive capabilities etc.)
Exclusion criteria
Exclusion criteria: - Underage patients - Hospital referral by doctor’s office or medical transport to the ER

| Secondary endpoints: - Consistency of the level and time of care recommended by SmED-Kontakt + with the assessment by the treating physicians in order to quantify the amount of over- and under-triage. - Quantification of the SmED-Kontakt+ recommendations regarding the different levels of care (emergency room, contract doctor, contract doctor teleconsultation (telephone or video)) - Quantification of the SmED-Kontakt+ recommendations regarding the different times of care (emergency, medical treatment as soon as possible, medical treatment within 24 hours, medical treatment not required within 24 hours) - Characteristics (demography, triage category, symptoms, diagnoses, whereabouts, etc.) stratified according to the recommended levels of care and time of care. - Descriptive analysis of the resources likely to be required for the patient’s treatment. - Descriptive analysis of the causes that require treatment in the emergency room. - Clinical endpoints of the emergency admission and, if applicable, subsequent inpatient stay (length of stay, mortality in hospital, stay in intensive care unit) - Comparison of the initial assessment categories according to the Manchester Triage System (MTS) or Leipzig Triage System (based on MTS) used in the routine for treatment prioritization in the emergency room and the recommendations by SmED-Kontakt+ - Resources required for the SmED-Kontakt+ assessment (personnel and time expenditure) | — |
