Inclusion criteria
Inclusion criteria: 1. hospital discharge with heart failure diagnosis (ICD-10-GM I50*, I11.0*, I13.0*, I42.0) within past 18 months. 2. =40 years of age. 3. Likelihood of Hospitalization =37.75% within the next 12 months.
Exclusion criteria
Exclusion criteria: - Chronic kidney disease stage 4 or 5 (N18.4 or N18.5) - cardiac assist device (Z95.80) - dementia (F00*-F03*) - dependency syndromes (F10.2, F11.2, F12.2, F13.2, F14.2, F15.2, F16.2, F19.2) - schizophrenia (F20-F29) - Alzheimer disease (G30.0, G30.1, G30.8, G30.9) - lack of legal capacity - nursing home residence or very high level of nursing care requirement - severely impaired hearing or sight
