Inclusion criteria: • Age > 18 Jahre • Patient from Poliklinik Veddel • at least 1 chronic diagnosis/ condition • living at home

Exclusion criteria

Exclusion criteria: • No Agreement • Dementia • lives/ moves in/ to long-term care facility • acute, severe course of disease
