Group 1: Patients with chronic disease and support needs in medication management and vital sign monitoring will use the Addison Care telecare platform over the course of 14 days and will be surveyed

Inclusion criteria
Inclusion criteria: - chronic health condition - planned patients transition from hospital to extramural care - three to nine drugs (regular intake of drugs, no status of hypermedication) - no moderate-to-severe cognitive impairment or severe psychiatric disease - no discharge with a newly diagnosed disease - 65 years or older - ability to speak and understand German language
Exclusion criteria
Exclusion criteria: - 64 of age and younger - more than ten drugs, status of hypermedication - moderate to severe cognitive impairment or severe psychiatric disease - no ability to speak and understand German language
