Inclusion criteria
Inclusion criteria: 70 years or older, at least 5 drugs used continuously, treatment at Düsseldorf University Hospital, written consent of the patient
Exclusion criteria
Exclusion criteria: legal care, inability to communicate due to language barriers or hearing loss, repeated inclusion due to recurrent treatments, visual impairment, cognitive impairment (minimum score of 4 on the Six Items Screening), inability to write, no written consent of the patient, instable clinical situation, patients with palliative care
