Inclusion criteria
Inclusion criteria: Inclusion criteria for residents: Min. of two unplanned acute medical services in the last 4 weeks (e.g. hospital admission or on-call service or outpatient treatment in the emergency room) OR: An unplanned acute medical service in the last 4 weeks (e.g. hospital admission or on-call service or outpatient treatment in the emergency room) and the relevance of the case based on the clinical assessment of the nursing home head nurse. Inclusion criteria for nursing (specialist) person: Has worked continuously in the care facility for at least 2 months. Inclusion criteria for relatives: Primary contact person (named by the resident) Inclusion criteria supervising physician: Involved in the medical care of the participant in the last 2 months Inclusion criteria for other relevant persons: Involved in the care of the participant in the last 2 months (e.g. other health care professionals, pastoral care)
Exclusion criteria
Exclusion criteria: Exclusion criteria for residents: Short-term care, Residents who have been living in the home for less than 2 months or who are currently not living in the home, dying within the next 3 months would not come as a surprise, Resident (or legal representative) refuses to question other people involved in (their own) care. Exclusion criteria for nursing (specialist) person: Not regularly involved in the care of the participants.
