Inclusion criteria
Inclusion criteria: The iCARE-PD care model is open to all Parkinson's patients and their care partners, taking into account the exclusion criteria. The age of the patients and their caregivers does not matter, as does the gender or origin of the patients and their relatives.
Exclusion criteria
Exclusion criteria: Patients who meet one or more of the following criteria will be excluded from the study: 1. Patients who are unable to give consent 2. Patients who are in-patient care in a nursing home 3. Patients whose autonomy is restricted due to motor symptoms (Hoehn & Yahr = 5) 4. Patients who are restricted in their autonomy due to significant dementia diagnosed by a doctor

| GOAL: To evaluate the care model in relation to the core components of the iCAREPD program: Criteria: 1. Creation of an integrated care model: o Number of visits or appointments per week o Duration of the visits or appointments o Follow-up activities after the clinical visits or appointments: number, mode of Communication and duration. 2. Supply navigation: o Use of every health resource in the iCARE-PD network o Percentage of subjects who followed the personal care path after 4 Months completed (%) 2. Technology-based supply: o Number of study participants who are able to fill out the information form in electronic data capture (EDC) to be completed in full o Data completeness of the patient information forms in the EDC GOAL: To evaluate the acceptance of the iCARE-PD care model by important stakeholders (Patients, care partners, experts in clinical care and other health professions) Criteria: • Questionnaire for assessing patient satisfaction (SAPS) • Survey of care-giving stakeholders (relatives, Parkinson's nurses, doctors) in Care model regarding your satisfaction using Likert scales (Healthcare Professional Satisfaction Questionnaire, Clinical Global Impression Change Scale) SECONDARY secondary target criteria: OBJECTIVE: Assessment of the change in central health parameters of the participants over time and assessment of the quality of care Criteria: • PDQ-8 index • PQoL Carer questionnaire • MDS-UPDRS (Part Ia, Ib, II and III) • Clinical Global Impression Change Scale • Chronic Disease Self-Efficacy Scale • PD-CHAPO model OBJECTIVE: To review the financial efficiency of the iCARE-PD care model Criteria: • Recording of the medical, non-medical and informal costs of PD patients in the iCARE-PD care model • Recording of the development costs of the iCARE-PD care model • Recording of the QALYs using the EQ-5D-5L questionnaire • Collection of socio-economic data (income, housing situation, etc.) | — |
