| The standardized questionnaire "Questionnaire for the Assessment of Exercise-Related Health Competence (BGK)" (Carl, Sudeck & Pfeifer, 2020) serves as the basis for the patient survey. The theoretical basis for the BGK is the model of physical activity-related health literacy, which describes the extent of health-effective physical activity via the 3 components of exercise, control and self-regulation competence (Pfeifer, Sudeck, Geidl & Tallner, 2013). Physical activity-related health literacy is considered an essential prerequisite for people to independently and self-determinedly lead an active lifestyle that is adequate according to their pre-existing conditions. The BGK is collected once by means of an online survey. | — |

| Secondary endpoints are a) patients' activity behavior b) patients' attitudes regarding physical activity (beliefs, fears, knowledge of health benefits). The questions were self-developed and tested for comprehensibility, relevance and completeness in a preliminary study. The secondary endpoints will be collected once by means of an online survey. | — |
