Inclusion criteria
Inclusion criteria: Receiving outpatient or residential care service during the intervention period; resident of a participating nursing home or receiving home health care from a participating outpatient facility.
Exclusion criteria
Exclusion criteria: Not a recipient of outpatient or residential care services; Not a resident of a participating nursing home or a recipient of services from an outpatient care facility.
