Inclusion criteria: coronary heart disease, chroinic heart failure, diabetes, hypertension

Self-rated health (SRH) was assessed using a well-established and validated single item measure. Participants were asked to estimate their SRH on a scale ranging from 0 (“very poor”) to 10 (“very good”). To assess emotional well-being and depressive symptoms, we used the World-Health-Organization-Five scale (WHO-5) assessment points 1. baseline, 2. post = 5 months later, 3. follow-up 7 months later	—
