Inclusion criteria

Inclusion criteria: 1. Age over 18 years. 2. Initial occlusion of the internal carotid artery or the proximal middle cerebral artery (M1- or the dominating M2-segment). 3. An ASPECTS of =8 points in the initial native cerebral CT OR a futile mechanical thrombectomy. 4. The first ultrasound examination can be done with 12 hours after symptom onset. 5. Aggreement to participate in this study by either the patient (written or under witness) or by a physician with later confirmation by the patient itself or a legal representive).

Exclusion criteria

Exclusion criteria: 1. Injury of the eyeball or the temporal bone. 2. Pregnant or breast-feeding patients.

1. Dynamics of the sonographic midline shift, of the opticus nerve sheath diameter, and the pupillary diameter as predictor for the development of a space occupying cerebral infarction. 2. Correct identification of those patients by ultrasound that will develop a space occupying cerebral infarction. 3. Assessment of the interrater aggreement between the initial ultrasound examiner and the central assessor who will repeat all measurements on short videorecordings of the ultrasound examination.
