Inclusion criteria

Inclusion criteria: Insured persons of the participating health insurance companies, who - are over 75 years old - are resident in the respective HOWOGE quarters or live in the postal codes 12681, 12689, 12687, 12679, 12685, 12619, 12629, 12627, 12683, 13053, 13055, 10369, 10367, 10365, 10315, 10317, 10319, 10318

Exclusion criteria

Exclusion criteria: - Persons who have a care level of 3 or higher - Diagnosis H91.3, H54.4, F10, F20, F30 and F31. - Persons without prescription

Design outcomes

Primary

MeasureTime frame
Unplanned (avoidable) hospital admissions and emergency doctor and rescue center contacts. These are recorded at the beginning of the intervention (baseline) and after 12 months (follow-up). They are recorded by means of secondary data analysis of routine health insurance data and a questionnaire survey.

Secondary

MeasureTime frame
- further use of health care: Family doctor contacts/contacts to the outpatient system, prescribed drugs, care costs, degrees of care and their development - Quality of life, perception of stress, feeling of security, social and health situation of the study participants - Satisfaction/assessment with the new form of care by the study participants - Frequency of contacts with the health office and their occasions - Remaining at home vs. home admission
