Inclusion criteria
Inclusion criteria: - =18 years - written consent has been obtained, - Full command of the German language - they are informed that his or her disease is not curable and has progressed and that the probability of surviving the disease is low - Surprise Question answered with “no” (and SPICT criteria are met)
Exclusion criteria
Exclusion criteria: > Person refuses to participate > No sufficient knowledge of German > The patient is not able to give written informed consent to participate in the study. > The patient is not informed that his/her disease is not curable and advanced and that the probability of surviving the disease is low. > Control group: SPICT criteria not met. > Intervention group: prognosis of 12 months (Surprise Question answered with "yes") > No consent / or consent later withdrawn
