Group 1: Part A: Patients with coronary heart disease (CHD) who have already been recruited and tested in MenDis-CHD I will be retested once in a follow-up study. Sociodemographic data will be collect

Inclusion criteria

Inclusion criteria: Part A: diagnosed CHD, above 18 years, sufficient German language skills Parts B and C: general practitioner in private practice in Cologne

Exclusion criteria

Exclusion criteria: severe mental and/or cognitive disease (suicidality, delirium etc.)

(A) To identify predictors of longitudinal pathways of care and health-related outcomes in patients with coronary heart disease (CHD) with/without mental or cognitive disorders (MCD), (B) to perform qualitative interviews to develop guidance for general practitioners (GP) with regard to the detection and care of MCD in CHD. Both (A) and (B) inform the design and implementation of a brief trigger question-based intervention in GPs (C) for both physician and patient (provider-patient trigger question technique (PPTQT), stimulating action towards guideline-based health care of MCD in patients with CHD. The study aims to analyse the following data after carrying out the intervention: 1) Organizational characteristics of GP practices and patient care (e.g. number of staff, opening hours, practice equipment). 2)Proportion of MCD diagnoses in CHD patients and MCD-specific diagnostic and therapeutic measures in CHD patients. 3)Operational processes during the treatment of CHD patients in PCP practices (number of all patients treated). 4)Perceived influence of organizational determinants on the implementation process, the perceived benefit of the intervention, and feasibility from the perspective of PCPs.	—
