Inclusion criteria

Inclusion criteria: Intervention group: Member of the orthopaedic specialist contract according to § 73c SGB V in Baden-Württemberg (Germany), adequate knowledge of German, medically confirmed diagnosis: osteoarthritis, osteoporosis, chronic back pain, rheumatism ; Control group: adequate knowledge of German, medically confirmed diagnosis (osteoarthritis, osteoporosis, chronic back pain, rheumatism), statutory health insurance member

Exclusion criteria

Exclusion criteria: Intervention group: Not a member of the orthopaedic specialist contract according to § 73c SGB V in Baden-Württemberg (Germany), no adequate knowledge of German, no medically diagnosed chronic orthopaedic diseases (see above) ; Control group: no adequate knowledge of German, no medically diagnosed chronic orthopaedic diseases (see above), not a statutory health insurance member
