Interventions

Group 1: Patients suffering from dementia and delirium. A patient-centered nurse will take care for them over the duration of their stay at the hospital, when required. Group 2: Relatives of patients

Inclusion criteria

Inclusion criteria: Patients of the University Hospital of Geriatrics and the Centre for Geriatric Traumatology, Cognitive impairment (dementia and/or delirium)

Exclusion criteria

Exclusion criteria: Patients of other units, who are treated on other wards; patients <65 years of age; patients without cognitive impairment; Patients with cognitive impairments who refuse to participate in the study after having been asked twice; Patients with cognitive disabilities whose legal guardian or health care proxy refuses to participate in the study

The primary endpoint of the study is the result of the Neuropsychiatric Inventar (NPI). At the beginning of the reference care and shortly before the patient is discharged.	—

1 Characteristics of the participants* (e.g. age, sex, year of birth, height and weight, state of health, visual impairment, hearing aid, degree of care), medication with dosage and duration of intake (start, in case of changes, at the end) 2 Activities of daily life, Barthel Index (at the beginning, once a week, at the end) 3 Fall risk, scale for fall risk assessment (at the beginning and end) 4 Fall events in hospital in the event of falls, fall protocol 5 Decubitus risk, decubitus risk scale (at the beginning and end) 6 Cognitive deficits, clock test (at the beginning and end) 7 Cognitive impairment, Mini Mental State Test (MMST), (at the beginning and end) 8 Depression, Geriatric Depression Scale (GDS) (at the beginning, at changes and at the end) 9 Eating, drinking and excretion behaviour throughout, import and export protocol, 10 Nutrition and drinking behaviour, continuous, Didier nutrition app 11 Nutritional status, Mini Nutritional Assessment (at the beginning and end) 12 Delir (duration and severity), 4 AT Test (at the beginning, during changes and at the end) 13 Balance, mobility, Tinetti test (at the beginning and end) 14 Balance, strength, timed up and go test and Demmi test (at the beginning and end) 15 Relatives are interviewed about their impressions of the development of their relative during their stay in hospital. 16 (Patient centered) nursing staff and the team of the wards are asked about their impressions of the effectiveness and team dynamics of reference nursing. Balance, mobility, Tinetti test, strength, Timed up and Go test and Demmi test will be used for retrospective comparison between the groups with and without reference care.	—
