Inclusion criteria
Inclusion criteria: DMP enrollment, willingness to participate, sufficient German language skills.
Exclusion criteria
Exclusion criteria: No ability to participate, consuming diseases, lack of German language skills.

| Mental level: Increase in health literacy (HLS questionnaire), increase in health-related attitudes (questionnaire); Behavioural level: improvement of self-management, improvement of nutritional and exercise behaviour (all questionnaires); Health status level: improved clinical outcomes (secondary data from the Association of Statutory Health Insurance Physicians); increase in health-related quality of life (SF12 questionnaire); All measurements are collected before (T0), after 9 months (T1) and after 18 months to conclude the intervention (T3).
