Inclusion criteria

Inclusion criteria: DMP enrollment, willingness to participate, sufficient German language skills.

Exclusion criteria

Exclusion criteria: No ability to participate, consuming diseases, lack of German language skills.

Design outcomes

Primary

MeasureTime frame
Number of hospital stays; check on secondary data of the health insurance funds; measurement period is the 18 months of intervention; secondary data are only available 9 months after the end of the intervention.

Secondary

MeasureTime frame
Mental level: Increase in health literacy (HLS questionnaire), increase in health-related attitudes (questionnaire); Behavioural level: improvement of self-management, improvement of nutritional and exercise behaviour (all questionnaires); Health status level: improved clinical outcomes (secondary data from the Association of Statutory Health Insurance Physicians); increase in health-related quality of life (SF12 questionnaire); All measurements are collected before (T0), after 9 months (T1) and after 18 months to conclude the intervention (T3). Diseases that prevent participation in group exercise activities; Missing/withdrawn consent to participate in the study.
