Group 1: Observational study - several disease related questionnaires as well as several functional diagnostic procedures will be done at baseline and during follow-up.

Inclusion criteria

Inclusion criteria: Patients with own smartphone and willingness to install an application

Exclusion criteria

Exclusion criteria: hospitalisation due to other reasons (other than COPD exacerbation) discharge from hospital with severe ventilatoric failure and non-invasive ventilation
