Conditions

Task substitution from general practitioners to non-physician practice staff. Patients with at least on chronic diseases, predominantly diabetes or hypertension will be included.

Inclusion criteria

Inclusion criteria: • general practitioners: currently practising general practitioners • general practitioners in training: general practitioners in training, contact to the Kompetenzzentrum Allgemeinmedizin of Instititute of general praactice and family medicine • health care assistant (HCA), specialised health care assistant (VERAH) or non-physician practice assistant (NäPa): completed qualification as VERAH or NäPa, currently working as VERAH/NäPa in a general practice; completed training as health care assistant and currently working in a general practice, only one HCA/VERAH/NäPa per practice • patients: age above 18 years, sufficient knowledge to speak and write German, Einwilligungsfähigkeit, at least one chronic disease leading to at least one permantly taking medication. We aim to include predominantly patients with diabetes mellitus or hypertension, which are regularly treated in their general practice for their disease • In order to ensure a broad overview of the attitudes and perceptions towards the research question we aim to recruit at only one person of the medical staff per practice

Exclusion criteria

Exclusion criteria: - participant withdraws informed consent or denies informed consent - no sufficient knowledge of the German language - less than 18 years old - no capacity for consent or severe illness that does not ensure a save study participation for the participant
