Inclusion criteria: Physician with practice in ambulatory care of all specialist groups.
Exclusion criteria
Exclusion criteria: Solely private medical practice; team size of the practice less than or equal to 2 persons

| The practices use an internal reporting and learning system, they systematically deal with their critical incidents (identifying, reporting, analyzing, deriving action, reviewing action). The practices exchange patient safety issues with other practices. These target criteria are detected via the process evaluation, i.e. by surveys during the intervention phase. | — |
