Inclusion criteria: Discharge in 1-3 days

Exclusion criteria

Exclusion criteria: Patients who are incapable of giving informed consent, without an authorized representative / supervisor / caregiver Patients not expected to survive the next 3 months Insufficient language skills in German

Cognitive change/ delirium after change of location (before transfer vs. day 3/7/90 after transfer (MoCA + 3D-CAM/CAM-S + Nu-DESC + FAM-CAM + indications of delirium in (third-party) anamnesis))

Falls, ADL, antipsychotic medication, etc. day 3/7/90; costs, mortality, institutionalization over the course of 3/12 months
