Group 1: Study participants who are accompanied by a reference person when they are discharged from one of the four centers, transported to their new place of residence and admitted to their new place

Inclusion criteria

Inclusion criteria: Discharge in 1-3 days

Exclusion criteria

Exclusion criteria: Patients who are incapable of giving informed consent, without an authorized representative / supervisor / caregiver Patients not expected to survive the next 3 months Insufficient language skills in German
