Interventions

Group 1: Intervention: Participants receive a 30-minute visit from a specialized palliative care nurse. The visit is followed by a 30-minute interdisciplinary telephone case conference by the patient

Inclusion criteria

Inclusion criteria: congestive heart failure (NYHA 3-4), dementia (GDS =4), or chronic obstructive pulmonary disease (Gold 3-4, group D), at least one consultation with the GP in the last three months, no support from SPHC services, no tumour diagnosis, signed consent form

Exclusion criteria

Exclusion criteria: congestive heart failure (NYHA <3, no hospitalisation in the last 12 months), dementia (GDS <4), or chronic obstructive pulmonary disease (Gold <3), no visit at the GP in the last three months, a current tumour diagnosis, current support from specialized ambulatory palliative care, no signed consent form
