Inclusion criteria: 1) Participation in Check-Up 35+ program at the general practitioner 2) Age > 18 years 3) Written informed consent

To set up an ideal schedule and road model for linking initial diagnosis to a treatment plan with in detail a. documentation of how further diagnostic steps, referral to a specialist and therapy guidance is performed in subjects with diagnosed chronic Hepatitis C b. Documentation of HCV treatment uptake and therapy adherence in subjects with diagnosed chronic hepatitis C

1) To evaluate the discriminative value of the risk questionnaire to identify subjects at risk for HCV 2) To evaluate advantages or disadvantages of a general screening compared to a risk adapted screening
