| Measure | Time frame |
|---|---|
| Functioning, measured with the Barthel Index (Mahoney & Barthle 1995; Schädler et al., 2012) at admission (T0) into the acute and transitional care (AAÜP) and at AAÜP discharge (T1). To measure functionality, the Barthel Index is a validated and common instrument. It includes 10 items that are rated on a 0, 5, 10 and 15 points Likert scale. A score of 100 points indicates maximum independence. Data Collection takes place with accurate monitoring the patients or interviewing patients, their families or nurses. | — |

| Measure | Time frame |
|---|---|
| Independence measured with the Degree of dependency on nursing care through the Resident Assessment Instrument, category "Minimum Data Set", Nursing Home Version at AAÜP entry (T0) and exit (T1) (Anliker et al., 2009). All items are collected through direct observation or through an interview with those affected, their relatives or caregivers. - Cognitive performance (MMS) - Delir (DOS) - Frailty (Frailty Index) - Mobility (Short Physical Performance Battery) - Fall self-efficacy | — |
