Inclusion criteria

Inclusion criteria: Age at least 65 years, hospitalization of an acute, chronic disease or an exacerbation.

Exclusion criteria

Exclusion criteria: Age < 65 years

Design outcomes

Primary

MeasureTime frame
The primary endpoint is mortality. This was collected 12 months after discharge from the hospital by telephone interview.
