Group 1: 1. Optimal medical treatment 2. Telemedical Monitoring of modifiable risk factors (Body weight, blood pressure, physical activity, blood glucose) via Smart Phone. 3. Telephone based coaching.

18 Years to 80 Years

Inclusion criteria

Inclusion criteria: Diagnosed with cardiorenal syndrome (NYHA II-III and CKD 3b-4) Heart or kidney related hospitalisation within the last 12 months, but not in the last 4 weeks Ability to comply with the instructions using the telemonitoring system Physical and mental ability to participate in the programm Internet access, email address Depression Score PHQ-9 < 15

Exclusion criteria

Exclusion criteria: Cancer within the last 3 years Dementia or uncontrolled psychological disease / depression PCI / CRT within the last 4 weeks Acute reduced hearing ability Language barriers Pregnancy / lactation

Primary

MeasureTime frame
Avoidance / reduction of heart and kidney related hospitalisation days over 12 months by evaluation of the discharge letter.

Secondary

MeasureTime frame
Evaluation after 12 months: Delay or avoid renal disease Progression (eGFR) Improving quality of life (QoL Score in the SF36 test) Physical activity (steps per day)
