2. Telemedical Monitoring of modifiable risk factors (Body weight, blood pressure, physical activity, blood glucose) via Smart Phone.
3. Telephone based coaching.

Inclusion criteria
Inclusion criteria: Diagnosed with cardiorenal syndrome (NYHA II-III and CKD 3b-4) Heart or kidney related hospitalisation within the last 12 months, but not in the last 4 weeks Ability to comply with the instructions using the telemonitoring system Physical and mental ability to participate in the programm Internet access, email address Depression Score PHQ-9 < 15
Exclusion criteria
Exclusion criteria: Cancer within the last 3 years Dementia or uncontrolled psychological disease / depression PCI / CRT within the last 4 weeks Acute reduced hearing ability Language barriers Pregnancy / lactation
