Conditions

The study will include Patients of the University Hospital Leipzig who are discharged from the palliative care center or the medical clinic and outpatient department I having a palliative diagnosis and a palliative treatment goal.

Interventions

Group 1: For all palliative patients who are being discharged home, demographic and other patient related data shall be collected using the electronic health file.

Inclusion criteria

Inclusion criteria: Patients of the University Hospital Leipzig who are discharged home from the palliative care center, the medical clinic radiotherapy and the medical clinic and the outpatient department with a palliative diagnosis and a palliative treatment goal, should be addressed and asked to participate.

Exclusion criteria

Exclusion criteria: Rejection of the Patient

MeasureTime frame
The number of rescue service contacts and proportion of inappropriate therapeutic measures are supposed to be recorded. Evaluation of the ambulance service contacts is carried out by identifying the patients and retrieving the Database of the integrated regional office of the fire department of Leipzig.
