Inclusion criteria

Inclusion criteria: Age > 65 years admittance to hospital due to acute or exacerbation of chronic disease

Exclusion criteria

Exclusion criteria: age < 65 years

MeasureTime frame
The primary endpoint is mortality. This was collected after 12 months after discharge from the hospital by telephone interview.
