Inclusion criteria

Inclusion criteria: Multimorbidity Age >65 years Agreement of patient Patient stayed in nephrological ward of Universitätsklinikum Köln Mpi at admission and dismissal

Exclusion criteria

Exclusion criteria: No multimorbidity Age <65 One or no MPI-calculation no agreement of patient

Difference in the Delta-MPI of the standard care group and the intervention group Calculation of the admission- and discharge-MPI, calculation of the delta-MPI, and statistical analysis if the delta is significantly different in the two study arms
