Group 1: On a single occasion, patients with polypharmacy will be questioned with the newly developed electronic tool "PolyPref" regarding their pharmacotherapy-related preferences (short-term and lon

licensed German version, www.iqvia.com/TSQM) and the instrument „Present Versus Future Health” (Case et al, Patient Educ Couns 2013

German translation carried out by our study group).

Inclusion criteria

Inclusion criteria: • Age =70 years • At least two chronic medical conditions • Regular intake of = 5 medications • Mentally and physically able to participate in the study (as judged by a study physician or the treating physician at Agaplesion Bethanien Hospital)

Exclusion criteria

Exclusion criteria: • MMSE =23 points • Known diagnosis of dementia • Insufficient knowledge of written and/or spoken German • No written informed consent
