Inclusion criteria
Inclusion criteria: Admission AAÜP and participation in the rehabilitative and transitional care programme (RÜP), German speaking, existing informed consent
Exclusion criteria
Exclusion criteria: If a dcoumented denial to use the data exists, the affected patients will be excluded.

| Functioning, measured with the Barthel Index (Mahoney & Barthel 1995; Schädler et al. 2012) at admission into the acute and transitional care(T0), at discharge (T1) and six months after discharge (T3). To measure funcionalty, the Barthel Index is a validated and common instrument. It includes 10 items that are rated on a 0, 5, 10 and 15 points Likert scale. A score of 100 points indicates maximum independence. Data collection takes place with accurate monitoring the patients or interviewing patients, their families or nurses | — |
