Inclusion criteria
Inclusion criteria: Inclusion criteria for geriatric patients: • Patients =70 Years with at least two geriatric feature complexes (i.e. = 2 points in the ANGELINA questionnaire from at least two different topic complexes) • sufficient German language skills • For patients who are being cared for in health care and/or have a consent requirement, the caregiver must also sign the consent form. Inclusion criteria for practices: • The doctor must be a member of the respective practice network. • be cash approved • Participants can be general practitioners of internal medicine, general practitioners and general practitioners.
Exclusion criteria
Exclusion criteria: Exclusion criteria for patients at the time of recruitment: • Patients living in inpatient care facilities • Patients with a terminal disease in the last phase of life and/or use of specialized palliative care (SAPV, hospice) • Patients with a bipolar disorder or other serious psychiatric illness. Exclusion criteria for the practices: • Exclusively private medical practices

| 1.) Determination at patient level: - Coping with daily household tasks measured with the IADL questionnaire - Mobility measured with "Timed up and go" - Falls detected with a fall log - Quality of life measured with WHOQOL-OLD/ and -BREF - Nutritional status measured with the MNA-Elderly - Determine treatment satisfaction of the geriatric patients in the intervention group as well as utilization of care services and associated costs. 2.) Determination at the level of affiliates and associates: - Load measured with the "Domestic care scale". - Determine the level of support - Record treatment satisfaction 3.) Determination at the level of the service provider: - Measuring the duration between the identification and implementation of a care need - Reduction of interface problems (cross-sectoral) between different service providers - Query satisfaction with the supply situation - Derive improved teamwork through delegation | — |
