Group 1: No interventions will be performed in this study. Study participants will answer one standardized questionnaire consisting of several scientifically validated and self-developed questionnaire

Inclusion criteria
Inclusion criteria: Age 18+, presence of a chronic or hereditary disease which results in physical impairment, German nursing care level minimum level 2, current process of ATD supply, which is planned or already completed, written informed consent
Exclusion criteria
Exclusion criteria: cognitive impairment, which results in the disability to fill out the questionnaires, ATD supplies for the improvement of a palliative situation with a expectable end of life within the following 3 months, no written informed consent

| The aim of the study is to examine how the ATD supply process including application and approval or rejection, the duration of the process of application until approval or rejection, and the possibility of the involvement of the patients in the selection and the range of function of the approved ATD supply affect the quality of life of patients. Part A: collection of sociodemographic date: living situation, status of care, health status, current ATD and frequency of usage, current support need, questions referring to the indicated ATD (reason for indication, funding, expectations to ATD), self-efficacy (validated, SWE), quality of life (validated, QUOLS), questions about current support concerning activities of daily living in the fields of communication, mobility, nursing, social engagement, patient-autonomy-need-index (validated, API-DM). Part B: received ATD: use and frequency of use, reason for acquisition, supply process, satisfaction with ATD and supply process (Quality of Assistive Technology Service Delivery Process, KWAZO) | — |
