Inclusion criteria: 1 Admission in one of the ccoperating cardiac Rehabilitation centers AND 2 LVEF of maximum 40 % after admission AND 3 written consent to participation (form) AND 4 written consent to data processing (form)

To measure how cardiac Rehabilitation in heart failure is able to comply with the GBA demands for German DMP programs from August 2018: Improvement in subcategories of Kansas City Cardiomyopathy Questionnaire (KCCQ) Score more than 20% during rehabilitation therapy (A/D) with stable results after 6m Improvement of 2 points in Hospital Anxiety and Depression Scale (HADSD) Score A and HADSD Score D between A/D Improvement of 20 Points or more in Barthel Score between A/D Improvement of 40 Watt Performance in Ergometer Training between A/D Increase in prognostic medication dose of 20% between A/D Hospital Transfer /Admission lower than 5% between A/D	—
