Inclusion criteria

Inclusion criteria: - Age = 65 years - multimorbidity (= 2 chronic diseases in need of treatment) - Patient or substitute able to consent - adequate language skills - grade of care <4

Exclusion criteria

Exclusion criteria: - Age = 4

Primary

MeasureTime frame
Rehospitalisation days (in-hospital days) in the 6 months after discharge

Secondary

MeasureTime frame
Assessment of whether the intervention in the study influences the following health indicators after 1, 3 and 6 months: A) length of hospitalization, B) number of existing preventive measures (preventive power, living will), C) development of the level of care, D) nursing home use, E) Number of drugs, F) presence of depression symptoms, G) measure of activity of the patient (Theraband, pedometer), H) quality of life (EQ-5D-5L), I) self-efficacy (RSES), J) satisfaction of the patient with the discharge management, K) satisfaction of the attending family doctors with the dismissal management and the team work, L) general practitioner's use of the contact offer to the Geriater after hospitalization.
