Inclusion criteria
Inclusion criteria: Patients are randomly selected from general practitioner (GP) charts and included if they have at least 3 chronic conditions and consulted their GP within the last completed quarter (i.e. 3 month period).
Exclusion criteria
Exclusion criteria: Exclusion criteria in preliminary qualitative study: 1. Insufficient ability to consent (e.g. dementia) 2. Insufficient ability to participate in focus groups (e.g. blindness, deafness) 3. Severe illness probably lethal within three months Poorly known to the GP 4. Residence in a nursing home 5. Insufficient ability to speak and read German Exclusion criteria for quantitative study: 1. Insufficient ability to consent (e.g. dementia) 2. Insufficient ability to participate in focus groups (e.g. blindness, deafness) 3. Severe illness probably lethal within three months Poorly known to the GP 4. Residence in a nursing home 5.
