Group 1: Early group (Patients who underwent surgical debridement within 12 hours of injury) Group 2: Late group (Patients who underwent surgical debridement at 12 or more hours from injury)

Inclusion criteria

Inclusion criteria: All adult, skeletally mature (as seen on radiographs) patients with grade 3 open fractures of the femur, tibia, or fibula, including intra-articular fractures, who provided informed and photographic consent.

Exclusion criteria

Exclusion criteria: Patients who had fractures involving the face, trunk, upper limb, spine, pelvis, patella, or foot; who underwent primary debridement elsewhere; who had unsalvageable limbs requiring urgent amputation and those who were unwilling to participate in the study.

Design outcomes

Primary

MeasureTime frame
Fractures which have not united and compare between both study groups. To evaluate fractures which have not united at the end of 9 months clinically (by examination for abnormal mobility, tenderness at the fracture site and inability to stand on 1 leg) and radiologically (presence of atleast 3 out of 4 healed bony cortices).

Secondary

MeasureTime frame
1. To evaluate infection rates at the end of 9 months following surgery. 2. Patient quality of life (functional scores SF-36 and SMFA) compared between both groups.
