Group 1: Usual care: Usual trans-sectorial care management for old and very old multi-disabled people at hospital discharge includes: 1. medical referral letter to the patient’s general practitioner
2. if indicated referral to in- or outpatient rehabilitation
3. if indicated referral to outpatient nursing service or nursing home. Group 2: Usual care plus LoChro Care: LoChro Care is following the ARIADNE principles for the management of multi-morbidity. The

Inclusion criteria: ISAR score >2

Exclusion criteria

Exclusion criteria: (1) terminal medical conditions (2) not having simple German language skills and (3) not residing in the region

Primary

MeasureTime frame
(1) functional health in terms of daily activities (WHODAS), (2) depressive symptoms (PHQ-9) Measurements at t0 (baseline), t1 (after 12 months) and t2 (after 18 months)

Secondary

MeasureTime frame
(3) satisfaction with care (PACIC), (4) resource utilisation, health care costs (FIMA) and (5) appropriateness of medication (START/STOPP) Measurements at t0 (baseline), t1 (after 12 months) and t2 (after 18 months)
