Inclusion criteria
Inclusion criteria: Clinicians: 1. member of rehabilitation team for one year, 2. active participation in interprofessional practice, 3. age >18 years, 4. sufficient German language skills Patients: 1. involvement in inpatient rehabilitation in final third of stay, in care of an intervention team, 2. age: 60 to 90 years, 3. sufficient language skills in the languages of patient questionnaires (German + 5 other languages), no cognitive impairments, existence of written consent
