Inclusion criteria

Inclusion criteria: 70 years and older; ISAR score> 2; Taking 5 or more medications; Pre-medication known; Main and other diagnoses known; Medication analysis performed: - Postoperative (control group) - pre- and postoperative (intervention group); Preparation and transmission of treatment recommendations pre-operative (applies only to the intervention group); at least one completed medication analysis including findings as well as the preparation and passing on of therapy recommendations; Post-operative Medication analysis and diagnosis of discharge medication; consent form

Exclusion criteria

Exclusion criteria: <70 years; <5 medicines; ISAR score <3; Premedication unknown; Main and secondary diagnoses unknown; no medication analysis performed; no preparation and transfer of treatment recommendations pre-operative (only for the intervention group); no consent given
