Inclusion criteria

Inclusion criteria: current inpatient hospital stay due to an acute coronary syndrome - at least 70 years old - informed consent signed - in the case of inclusion, it is to be assumed that the patient can be discharged to the family at the end of the stay in the hospital

Exclusion criteria

Exclusion criteria: - the presence of a malignant disease with a life expectancy within the observation period, - in the opinion of the attending doctor expected compliance problems, - inability to deal with telemedicine equipment or telephone communication, - concurrent participation in other studies, - implanted assist device as well as listing for heart transplantation.

Primary

MeasureTime frame
Mortality of any cause after 12 months, FU with questionnaire
