Interventions

Group 1: 6-months follow up of nursing home residents with heart failure: standard clinical examination 1 week, 1 month and 3 months after diagnosis confirmation and in case of deterioration or change

Inclusion criteria

Inclusion criteria: For screening: nursing home resident in assigned nursing homes For clinical trial: nursing home resident with heart failure in assigned nursing homes

Exclusion criteria

Exclusion criteria: 1. A life expectancy less than 6 months for other reasons than HF, assessed by »surprise question« 2. Patients who will not sign informed consent or whose legal guardian will not sign informed consent.

Design outcomes

Primary

MeasureTime frame
The observed outcome for primary observed events will be the time to exacerbation of HF, defined as a need for intravenous diuretics or a need for emergency or out-of-hours intervention or a need for unplanned hospitalization for non-injury causes.

Secondary

MeasureTime frame
For secondary observed events, outcomes will be the time to whichever individual observed event, the change in quality of life and functional state after 6 months, number of days in hospital, out of hospital and death independent of the cause.
