Inclusion criteria

Inclusion criteria: Residents of a long-term care home in the intervention counties, who are insured with a statutory health insurance. Nurses and medical doctors from the participating long-term care homes.

Exclusion criteria

Exclusion criteria: Residents of the long-term care homes: Dementia (as an exclusion criterion for participation in the questionnaire survey and the focus group, but not as exclusion criterion for the trial) as well as a residence time in the long-term care home below the minimum time of 3 months. Nurses and physicians: no exclusion criteria

Costs of health care (cost-related routine data of the statutory health insurance): pre-post measurement. Quality of care experienced by the resident (questionnaire for the residents, group interviews with residents): data collection once in the intervention period. Usefulness assessed by the practitioner (questionnaire for the practitioner, group interviews with practitioners, audits): pre-post measurement. Data for the intervention group are included from the period: 1.1.2017-30.9.2019. Data for the control group are included from the period: 1.1.2017-30.9.2019
