Group 1: We will rekruit patients older than 75 years in the emergency department of Uniclinic in Cologne. The Patients of the First Group will be checked with the CGA Screening Questions with the M

Inclusion criteria

Inclusion criteria: 1. The Patients, which were bevor 1942 born. For the second phase patients, which were born bevor 1944.

Exclusion criteria

Exclusion criteria: 1. The Patients, which mentional don't can to sign up the consent.

Design outcomes

Primary

Measure	Time frame
Primary Outcomes are: Rate of Hospitalisation, Rehospitalisation, MPI (Multidimesional Prognostic Index), Institutionalisation within two years after the first contact.	—

Secondary

Measure	Time frame
The Patients will receive the Questions to their Life with the Geriatric Assessment with MPI. Than we will contact the Patients per phone monthly within the two years with the Quastions of our Visits. The secondary outcomes are: length of hospital stay, time to first rehospitalization, institutionalization, mortality rate, patient’s quality of life.	—
